Fill all fields in CAPITAL letters as per your Notary appointment, certificates and identity documents. Avoid overwriting.
Paste photographs where required and attach all attested copies and PDF printouts as per the “Before You Start” section.
Make payment as per the form instructions and submit the complete set to the Association (Head Office / designated office).
